Transcript Requests

To order a copy of your transcript, please follow the steps below.

Download the Official Transcript Request Form, print, sign, and mail it Little Flower Catholic High School, 1000 West Lycoming Street, Philadelphia, PA 19140, Attn: Guidance Department. 

Please be sure to include your $8.00 payment in the form of a check payable to Little Flower High School.

Transcripts are sent directly to institutions, not individuals.
